Welcome to the New Mexico Comparative Formulary Web Application

Home Formulary Benefits Reports


The intent of this resource is to provide patients, pharmacists, and healthcare providers with an easily accessible, up-to-date comparative formulary and drug benefit design. This is not intended to be a listing of all drug products. It is only a comparison of preferred formulary drug products from participating managed care organizations. For further explanation, please refer to the Benefits/Exclusions section or the plan's web site.
   
Home Click on this button to return to this screen from anywhere in the application.
   
Formulary Click on this button to display the Comparative Formulary Selection form. You may select drugs by general therapeutic class, specific therapeutic class, generic name, brand name, or a part of the name.
   
Benefits Click on this button to display the Benefits/Exclusions matrix by Organization Group.
   
Login Click on this button to display the Formulary Manager Login form. (Used by Formulary Managers to maintain the Comparative Formulary and Benefits data.)
   
Reports Click on this button to display or download Comparative Formulary Application Reports for printing or archival storage. A simplified version of the Formulary data may be downloaded to your PDA or notebook computer for off-line reference.
   
Survey Click on this button to display the Comparative Formulary Application Survey. (Help us make the Comparative Formulary better and easier to use.)

This list is not all-inclusive nor does it imply a guarantee of coverage. In addition, coverage for some drugs listed may be limited to specific dosage forms and/or strengths. Substitution of a generic product for a brand name drug is mandatory when a generic equivalent is available. If a member requests the brand name drug in this situation, a pharmacy exception may be required and the member must pay the difference in cost between the generic and branded versions.
